I realize that playing ice hockey is dangerous and may result in injury or death. I take all responsibility upon myself for any injury, loss or damage. I am voluntarily participating in these activities with knowledge of the danger and hereby accept any and all risks of injury or death.

In consideration of being permitted to play ice hockey, participate in any practice, or participate in any event sponsored by Roxanne Taylor and/or the Top Shelf Hockey league, I hereby agree that I, my heirs and assigns, will not make any claim against, sue, attach or prosecute Roxanne Taylor, the Top Shelf Hockey league and any of their participants, agents, employees, sponsors, lessors, and/or facilities for any liability to the undersigned for any and all loss or damage and any claim or demands thereof on account of any injury to my person or property or resulting in death of the undersigned, whether caused by the negligence or other acts, however caused of the said Roxanne Taylor and/or the Top Shelf Hockey league or otherwise while the undersigned is participating in any ice hockey game, practice or event organized by the said Roxanne Taylor and/or the Top Shelf Hockey League.

I hereby assume full responsibility for any risk of bodily injury, death or property damage due to the negligence of said releases or otherwise while participating in, observing, practicing, officiating, or working for any purpose relating to the Top Shelf Hockey league. I also hereby release and discharge said Roxanne Taylor, the Top Shelf Hockey league and affiliated organizations from all actions, claims or demands, I, my heirs or assigns now have or hereafter may have for injury or damage resulting from my participation in ice hockey.

As a participant in the Top Shelf Hockey League I pledge to:

1. Treat other participants with respect at all times.
2. Cooperate with other participants and officials at all times.
3. Never physically hurt or cause harm to another participant or official.
4. Listen to, and follow, all given instructions/directions from officials.
5. Ask the officials, in a calm manor, to explain any rules I do not understand.
6. Participate in a sportsmanlike and safe manner at all times.
7. Read and review the league rules and policies and adhere to them at all times.

If I cannot participate according to this Code of Conduct, then depending on the seriousness of the offense(s), any or all of the following consequences may occur:

a. Verbal warning
b. Temporary removal from game
c. Temporary suspension from league play (no refund of league fees)
d. Expulsion from the league entirely (no refund of league fees)

Sub rules:

1. Subs must be approved by league.
2. Subs should be of equal skill level to player who is out.
3. Only full time, rostered players are guaranteed a spot in playoffs and/or championships. If subs are needed they MUST be approved by the league or provided by the league. Failure to abide by this rule will result in forfeiture of the game. (Subs in championship games are not awarded prizes should the team win).

I have read and voluntarily agree to this release and waiver of liability and indemnity agreement and further agree that no oral representation, statement of inducement apart from the foregoing written agreement have been made to me by any person.  I also agree/contract to pay league fees associated with my registration.
